Do I need this mod for Minecraft 1.16.5 or below?

No, you don't.

Balm did not exist back then, so naturally all mods that depend on Balm did not depend on it back then.

There is no Balm for those versions because it is not necessary, the mods will run without it. Balm is only required in Minecraft 1.17 and above.

I accidentally ran /balm dev, how do I undo it?

Run the following commands:

/gamerule doDaylightCycle true

/gamerule doWeatherCycle true

/gamerule keepInventory false

/gamerule doInsomnia true

/gamerule mobGriefing true

/gamerule doTraderSpawning true

/difficulty easy

or whichever difficulty / game rules you had setup for your world.
